Other Student Information

  • Student/parent instructions for obtaining a work permit. Please read all instructions carefully and completely! 

    • Your Parent/Guardian must complete and sign the "Student/Applicant Information" portion of the Application for Minor Work Permit form.
    • Your Employer must complete the "Pledge of Employer'' portion of the Application for Minor Work Permit form. The Employer must include its Employer Tax ID Number (9 digits). This is mandatory.
    • Your Physician must complete the "Physician's Certificate for Minor Work Permit" portion of the Application for Minor Work Permit form.
      • Also Note: A Sports Physical form can be submitted in lieu of having the "Physician's Certificate" portion of the Application being completed by the Physician. The Sports Physical must be dated one year or less from the date of submission of the Application for Minor Work Permit to Xenia High School.
      • To obtain a copy of the Sports Physical, please report to the Athletic Trainer's Office (located in the Athletic Director's office in the blue building behind the Xenia High School in the staff parking lot) - immediately following the end of the school day.
    • Any one of the following options of identification for the student must be submitted with the completed application:
      • Birth Certificate/Record
      • Driver's license
      • Insurance Policy
      • Passport
      • Probate Court Record
      • Religious Record
    • The completed Application For Minor Work Permit and student identification noted above must be submitted to the Main Office at Xenia High School at 303 Kinsey Road, Xenia, Ohio. This Application must be properly completed to enable a Work Permit to be submitted to the State of Ohio for approval.
    • A new Work Permit must be issued with each change of employment.
    • Two simultaneous Work Permits can only be issued with the prior approval of the Xenia Community Schools' High School Principal and District Superintendent.

  • Parents/Guardians of 2024-25 Grade 12 Students

    Hello. The Ohio Department of Health requires your child to receive new  immunizations prior to the start of 12th grade. Your child will be required to receive two doses of the MCV4 -meningococcal vaccine which must be administered prior to entry of school in the fall of 2024. One dose should have been received prior to 7th grade. 

    We are encouraging you to schedule your appointment at this time to avoid the rush in the fall and your child’s exclusion. Failure to provide records will result in your child being excluded in the fall. Absences related to exclusion are unexcused. Please contact your child’s medical provider to schedule their immunizations. The Dayton Children’s Health and Wellness Clinic at Warner Middle School can provide your child’s immunizations at school. School transportation to the clinic can also be provided from Xenia High School. Please contact them at 937-641-4623 to schedule an appointment.  

    If you would like more information or exemption paperwork please contact the school clinic 937-372-6983/Extension 8411. Upon completion, please provide the school clinic an updated copy of immunizations. Immunization records can be uploaded into Final Forms or faxed to 937-352-4450.

    Immunizations per ORC 3313.671

    Unless exempted from the immunization requirements, a child cannot be permitted to remain in school for more than 14 days unless the child presents written evidence satisfactory to the school of having received the required immunizations or being in the process of receiving the required immunizations.
